Resident Care Conference

Tuesday, July 31, 2018
Mom was sitting with a group in the fish tank room.  They were having morning coffee.  She was in a wheelchair.  I took her away.  We did a puzzle together.  I put most of the pieces together and let her put the last two corners in.  She was pleased with herself.  She drank some of her coffee.  I brought her back to the group.  She was holding her mug.
I went to the Resident Care Conference.  Mom's doctor did not show up.  The nurse said that mom was incontinent.  Her weight is at 54.2 kg.  She wanders.  No restraints are used.  She is mostly nonverbal.  She has anxiety and is constipated.  She needs to be cued for everything.  She seems to have a pain in her lower back.  The pharmacy lady wants Dr. Cameron to reassess mom.  She suspects Parkinson's.  Mom is on Donepezil 10mg and Paroxetine (antidepressant) 20 mg,
and trazodone (for sleep) 50 mg. 
Mom is not eating much at all.  They are giving her Boost and another high calorie beverage.
Mom is very active in her programs.  She likes physical activity. 
After the meeting, I went to see mom.  She was doing arm exercises.  She wasn't able to follow.  She had no clue.  I had to put her arms on her knees.  She was enjoying the activity.
Then, we went to eat lunch.  Mom ate almost nothing.  I took her back to the room to toilet her and try and feed her more.  She ate very little.  She drank all her Boost.
She did a big poop in the toilet.  Hurray! 
I took her to participate in a gardening activity.  It was quite windy and some residents complained about the cold.  I asked mom if she wanted to come inside, but insisted on staying with the group so I brought her a jacket.  Finally, she said she wanted to go inside.  It was close to afternoon coffee time.  There was a mass exodus.  It was too windy.  I sat Donna with mom and left.

Mom was sitting with Donna when I arrived.  Mom was in a wheelchair. Her food was already served, but she hardly ate any of it.  I was only able to give her a very few small spoonfuls.  I think she drank her Boost.  She fell asleep several times during the meal.
After dinner, I took her juice, milk, my leftover tapioca pudding, a banana I got from the kitchen and mom's leftover pureed peach to her room.
Mom wanted to play the balloon activity so I let her do that.
I tidied her closet and drawers.
I found mouse poop in and around her closet.
I got disinfectant wipes and wiped out the closet and floor around the closet.
I moved her knit blanket and crocheted pillow cushions to the top of her closet.
I found a couple of pairs of socks that did not belong to her.
After the balloon activity, I brought mom back to the room to feed her.  She ate some of the food.
She doesn't seem to be able to chew anymore.
She wouldn't let me brush her teeth.
I got her up from the wheelchair, but she sat back down again.
I didn't attempt to toilet her.
I brought her back out to the nurses station for hot cocoa.  She didn't want to drink hot cocoa.
It's so hot.  She was wearing a long sleeve thick cotton t-shirt.

Another good day

Mom and Donna were already seated at the table.  Mom and Donna were having an argument about something.  I think mom was touching things on the table because she was bored.  She had consumed all her beverages.  I tried to take mom to the washroom but she wouldn't budge.  She was hunched and giving me the evil eye. 
Dinner was Greek chicken, cauliflower and rice.  Dessert was a fruit cocktail.
The server gave mom and orange.  Mom ate one section.  Mom ate her food today.  She fed herself a bit and I helped her for most.  I was just happy that she was eating her food.
I took her back to the room after dinner to toilet her.  She just peed.  There was a slight stain on her diaper.  I brushed her teeth and washed her face while she was on the toilet.  I got her to comb her hair at the back.
We played card bingo.  Mom was actually pretty with it.  She can still recognize suits.  She was more cooperative than usual in turning over the cards.  It was Michael, Donna, me and mom today.  I saw Shanta but she was in her room.  I asked Zorka and Lorne, but they didn't want to play.
After cards, I took mom back to the room to toilet her, but nothing came out.
She drank a cup of milk leftover from dinner and I gave her some banana.  She ate half.

A good day

Mom was sitting at the dining table.  She had already consumed her beverages.  Donna and Edward were seated.  Mom got served first, but she wouldn't touch anything.  She kept insisting I eat her food.  I got a banana while I was waiting for my spaghetti dinner.  Mom was eyeing the banana.  I peeled it for her, but she wouldn't take a bite.  I broke a piece off and put it in her mouth.  Then I gave her the banana.  She was eating it.
I was enjoying my spaghetti.  I tried her pureed food and the spaghetti sauce was good.  She ate most of the banana.  I tried to feed her some pudding, but she would have none of it.  She wouldn't have any of my spaghetti either.  I was going to take her back to her room, but she seemed interested in her food.  I was able to feed her more than half of her pureed food.  It was a good day.
I tried to get mom to take a poop.  She knew something needed to get out, but it just wasn't coming.  The aide told me that mom was constipated for two days and that she had been given something to make her poop.  I brushed mom's teeth and wiped her face while she was waiting on the toilet.  Her teeth don't look so good.
We played card bingo and she participated.  She was even able to distinguish the suit.
Mom was sitting upright and didn't seem so still today.
I got Joan to play with us because Lirong wasn't around.

Mom is stiff

Mom was sitting at the table with Donna when I arrived.  I took mom to her room to toilet her.
She had a dirty diaper.  She took a poop too.  I cleaned her up and took her back to the dining table.  She was so stiff.  She was one of the last ones to be served.  I gave her some of my food, which she ate.  She didn't touch any of her food.  Edward didn't want to eat his dinner.  I told him it was chicken.  He thought it was potato.  Finally, he tried it and ate it.  I cut up Donna's chicken for her.
Jacquie said they had an ice-cream party this afternoon.  She said that Donna said she was going to try mom's sherbet and ate it.
After dinner, I took Donna to the fish tank room, but she wanted to go back to her room. I made several trips back and forth to mom's room with her beverages and dessert.  I sat mom and Donna by the window so that they could look outside and see the roses.  Mom ate her dessert and drank her coffee creams.  I gave mom a banana and took Donna to the Fraser room so that she could take part in the casino game. When I came back, mom had not eaten the banana.  I broke off a piece and put it in her mouth.  She smiled.  She liked the banana, but she just didn't know how to eat it.  I put it to her mouth and she took small bites.  I gave her sips of her Boost.  After she had eaten half the banana, I put the head set on her and we went for a walk.
There is a lock on the gate that goes to the back garden.  We walked there and sat for a bit.  I couldn't figure out the code to get to the dog park.  We sat on a bench and mom noticed a bunch of ants crawling on the ground.  Her eyesight is pretty good. We came back and walked to the Fraser room and to the outside deck to see all the plants that were growing there.  I took mom to the front entrance and we sat for a bit there as well.  I was just trying to get mom to get some exercise.  She's so still.
We went back to the room and mom finished her banana and Boost.  I brushed her teeth.  She swallowed the water that she was supposed to rinse with.  The second time I brushed her teeth, she swished and spit, but the 3rd time, she drank again.  Poor mom.  I took her for a final pee.
I left her in the fish tank room with the other ladies.  Donna is on a diet so can't participate.  She was being toileted.

Mom is cranky

Mom was sitting at the dining room table by herself when I arrived.  I took her to her room to toilet her, but I met the aide and she told me that she had already toileted mom.  Mom had done a poop.
I took mom back to the dining room.  Donna and Edward were seated and Jacquie arrived a little later.
It was Edward's birthday on Friday, July 13th.  His family had brought a cake to share with everyone after lunch.  Edward turned 93.
Mom fell asleep at the dining table.  I let her sleep for about five minutes and then I woke her up.  She kept saying she didn't like the food.  Mom fed herself about a third of her food.  I gave her some of my pork. 
After dinner, I took the Boost, milk, egg custard and banana back to the room.  I took Donna to sit in front of the fish tank.  Mom was drinking her apple juice when I got back.  I took mom and the apple juice back to the room.  She finished drinking the apple juice.  I got her to drink some of her milk.
I toileted her and washed her face and hands.
Rong (the Chinese lady) was already sitting with Michael when we arrived.  I got Donna and mom seated and we started to play.  Alan brought Shanta to join us. Mom was cranky and didn't really participate in the card game.  After the card game she didn't want to get up from her chair.
After the game, I took mom back to the room and gave her the Boost to drink and the egg custard.  I went to fetch Donna.  She is on a diet and is not supposed to have hot cocoa.  Mom ate most of the egg custard and most of the Boost.
I asked mom if her throat hurt or her teeth hurt.  I don't know if she understands.  She grimaces when she swallows.
I took mom and the Boost and sat her with the other Chinese lady by the nurses station.

A good day

Mom was sitting at the dining table when I arrived.  Donna and Jacquie were there too.  Edward had fallen asleep by the television in the fish tank room.  Shepherd's pie tonight.  It's not as good as it used to be.  After we told a few aides about Edward, one of them went to go and fetch him.  Mom was just playing with her food.  It was like she was pretending to make something for me and then she offered it to me.  I gave her the leftovers of my food.  She started to play with that food too.  I told her that she should eat it, but she seemed puzzled.  I put the spoon in her hand, but she still didn't feed herself.  Finally she let me feed her.  She said it was good.  I felt so relieved.  She ate all my leftovers and a glass of milk. 
I took her to the washroom and she immediately did a huge wet fart, but then she did a poop too.  Hurray!  Her diaper was soiled with a little poop and a lot of pee.  She said something stinks.  I changed her diaper and wiped her bum.  She was very cooperative. 
I brushed her teeth, wiped her face, got her to comb her hair.  She was concerned about her teeth.  She's missing two of them in the front upper left area.
I told her that's because she is not brushing her teeth.
We played cards sans Shanta.  I think Alan is avoiding me.  Hurray!
Michael, Donna, mom and the Chinese lady played.  Joe joined us later.  Zorka wanted to play too.  I guess I could have added her too.  I just didn't think of it.
After the game, Chris wheeled Donna to the nurses' station and Zorka went too.  I asked the other Chinese lady to sit with mom.  Her daughters said that she thought mom was a beautiful and elegant lady.  How nice!

Dr. Cameron

Dr. Cameron called me today.
He's going to change mom's medication.
He said she is not sleeping well at night.
He's going to increase some medication.
He's also concerned about her anxiety.
I said I didn't want her to be dopey.
I told him mom had constipation.
I asked for him to give mom something like Restoralax in her apple juice every second day.
I also asked for mom to get finger foods.
Also, mom is refusing her Tylenol.  I said to just give her what she'll take.
He also wants me to sign a MOST form.  I think it is a no CPR form.

Sun Dresses

When I arrived at 5:35 p.m., mom was still sitting at the table.  Her food was untouched.  She refused to eat anything.  I took her milk and her Boost back to her room with her.  I toileted her.  I cleaned her finger nails.  We went to play card bingo.
Donna had already been put to bed.  She had had a big bowel movement.
Michael dealed, Shanta and mom and I played.  Joe was being toileted so he couldn't come.  The Chinese lady that played with us yesterday came and joined us.  Her daughters were happy.  The Chinese lady was happy.
After cards, I went to the kitchen and asked for a banana.
I got mom to eat the banana and drink the milk.
I left her with a different Chinese lady for hot chocolate.
Hiroko had given me a couple of sun dresses.  I showed mom and she said they were nice, but when I offered to give them to her she said I should keep them.

Hot cocoa

Mom and Donna were sitting together at the dining table when I arrived.  I took mom to the washroom.  Her fingernails looked dirty.  I asked the nurse and she told me that mom had not had a bowel movement today. When I went to check on mom, I saw a poop coming out of her bum.  It wasn't much, but it was a poop.  I cleaned her up and changed her diaper because it stunk.  I cleaned her finger nails with the nailbrush Charlene gave me.
Mom drank most of her Boost.  She fed herself some of her dinner.  I gave her two bite-size pieces of chicken and she ate those.  After dinner, I took her dessert, a banana and dumped her milk in with the Boost.  I got Donna to hold the milk/Boost.  I told her not to drink it because it was my mom's but she said she tasted it.  I got mom to hold the banana and the dessert because I needed to push Donna in the wheelchair. 
I toileted mom again.  She peed.  I washed her face and combed her hair.
I went through mom's photobook and got mom to drink her beverage.
Then, we all went to play card bingo.  Mom was reticent at first, but then she began to participate.  She didn't want to sit beside Michael.  Shanta did not join us.  A Chinese lady joined us.  She didn't have good use of her hands, but she could recognize suits.
I took the girls back to the room.  I put the head set on mom and we ate a banana together.  She ate it all.  I took mom for a little walk.  The aide came and toileted Donna and got her ready for bed.
I took mom for hot cocoa and a cheese sandwich.  Mom ate some of the cheese sandwich.  Zorka was bent on getting mom to speak English.  I tried to explain that mom had a degenerative brain disease, but Zorka had a hard time understanding.  She kept on speaking to mom in a loud voice.  Mom was mad.  She kept saying I don't understand.  At one point she said that she didn't understand because she was stupid and then she said that she wanted to die.  Poor mom. 
I had to take mom away from Zorka because even though Zorka was trying to be helpful, she was upsetting mom. 
Mom asked if she could bring the hot cocoa with her.  When we got back to her room it was dark, Joan was already sleeping.  Donna was still awake.  I asked Donna if I could turn on the lights and she was fine with it.  I sat mom down on the bed and showed her the hot cocoa.  I gave her a big hug.  She looked happy.

Finger nail brush

Mom was seated at the dining room table and had already been served.  Larry was trying to feed her, but she would have none of it.  Jacquie was eating at another table.  Edward had already been served.  Donna and I were waiting impatiently for our meals.  I put the spoon in mom's hand, but she just played with her food.  Eventually she let me feed her.  I was surprised, but happy.
Alan told me he had a plan to bring in someone from the Buddhist temple to visit with mom.  He asked me to speak with him.  I didn't say anything.
After dinner, I took mom and Donna back to the room.
I toilet mom.  She had poop stains on her diaper so I changed it.  I trimmed her bangs.  I gave her some Boost to drink and she drank it all.  She had what looked to be poop in her nails.  It looked like someone had already tried to clean it.  I soaked mom's nails in warm soapy water in a paper cup.  I used the soft plastic brush that Charlene had given mom.  It worked more or less.
I asked Zorka to join us for card bingo, but she said she was too tired.
Michael, mom, Donna, me and Chris played.  I forgot to order a dinner for him last Wednesday, but they managed to scrape something together for him.
Mom was pretty on task today.  She flipped over most of her cards with cueing.
After cards we took mom back to the room.  We left Donna with Zorka.
I got mom to eat some banana and a few sips of apple juice.  I brushed mom's teeth.
I asked Chris to talk to Alan, but Chris didn't want to talk to him either.
He said girls hit on him all the time and touch his arms and stomach.

Bingo

Mom, Donna and Edward were already seated when I arrived.
I remembered to change mom's calendar.  Zorka had lunch in bed today.
Jacquie arrived for lunch.  We had hamburgers.  Not so good.  Jacquie ordered egg sandwiches instead.  Next time I will too.
Mom hardly ate any of her food.  I got her to drink her beverages.
She seems to eat better when she is not at the dining table.
I put the head set on her.
Edward hardly ate anything.  He kept nodding off.
I changed mom's diaper.  It just had pee.
I took mom and Donna to play bingo.

Canada Day

Rat!  I just remembered I forgot to change mom's calendar.
I did remember to bring a cheque for meal tickets.  Also, a fingernail brush that Charlene gave me for mom.  I brought mom's walkman.  Turned out that it was the ear phones that were not working.  I used my ipod nano ear buds.  Mom enjoyed the music.
No one was at the dining table when I arrived.  Mom was sitting with Zorka in the fish tank room.  I took her back to her room to toilet her.  Her diaper was clean.  She just peed.  Donna was lying in bed.  She didn't know why she was there.
I took mom back to the dining room.  Edward was there, but Jacquie wasn't there.
We had ham, zucchini and peas and scalloped potatoes.  Mom really enjoyed her apple juice.  She wanted to drink the extra apple juice, but I told her that she had to eat her dinner first.  She ate about half.  She ate all her pureed meat and just a small portion of her pureed veggies.  She ate some of her Jello.  She drank all her Boost and some of her milk and the extra apple juice.
I took her back to the room to toilet her again, but nothing came out. I brushed her teeth, combed her hair, etc.  I took mom for a bit of a walk with the walkman. 
We played card bingo.  I invited Zorka.  It was Michael, Shanta, Zorka and Mom.  As usual mom didn't have a clue, but she was quite talkative.
After the game, I took mom back to the room to toilet her.  She peed a little, but still no poop.  Apparently she hadn't pooped all day. 
We shared an orange.  Mom only ate two segments.
Mom didn't want to stay with Donna so I took her back to the fish tank room.
Shanta was still there so asked her if she wanted to go to her room.  I was taking Shanta back to her room when her son showed up.  He said that the Japanese man had died.  I was surprised because he seemed fine the last time I saw him.  He said that the daughters were worried about the mom now because she is in the same facility.  They are worried about the quality of care.  Mom was growing impatient.  I don't think she likes Alan.  Neither do I.  So I told him we had to be off.  I took Shanta back to her room.  The door was shut so I opened it and mom wheeled Shanta in and then closed the door.  I opened the door so that we could leave and Shanta said she wanted me to leave the door open. I took mom to the washroom, but she said it wasn't necessary. We came back to the fish tank room and we sat with the Chinese lady.  The aide said that mom could have some egg sandwich if I sat with her.  Mom enjoyed the egg sandwich.  I also gave her the banana from dinner and she ate all of that too.  She drank her hot cocoa as well.  I moved mom over to sit with Zorka and then I left.
I saw Jacquie and another lady coming out of the Union Jack Pub.  They went to Tim Horton's to plug in Jacquie's wheelchair.
